Pause in Training Request — this form requests a Leave of Absence from your flight training program. Your request will be reviewed by our SEVIS agents (Lazaro Gonzalez, enrollment@flyingacademy.com).
Important SEVIS rules before you start:
Leaves of 4 days or less — no I-20 action required, but you will need a SEVIS agent (DSO) signature on page 2 of your I-20 before you travel.
Leaves more than 4 days — your I-20 will be terminated under "Authorized Early Withdrawal" and you must submit a SEVIS Reactivation requestat least 1 week before you plan to return to the U.S.
Leaves more than 5 months — your I-20 is automatically terminated and cannot be reactivated. You will need a new I-20 and a new visa interview.
Medical leaves require a signed letter from a licensed physician.

Required for ALL pause requests — regardless of leave length (even ≤ 4 days). Your SEVIS agent cannot process your request without these acknowledgements, your signature, and your signed I-20.
